Subpart A – General Provisions

"Medical information" means information on a person's medical history or condition obtained directly or indirectly from a licensed physician, medical practitioner, hospital, clinic, or other medical or medically related facility.

“Personal information” means any information that identifies, relates to, describes, or is capable of being associated with, a particular individual, including his or her name, signature, medical information, or health insurance information. “Personal information” does not include publicly available information that is lawfully made available to the general public from federal, state, or local government records.

“Personal information” as used in this section means any information that when it was disclosed identified, described, or was able to be associated with an individual and includes all of the following: height; weight; medical condition; drugs, therapies, or medical products or equipment used.

For purposes of records for the Board of Pilot Commissioners, "personal information" means information, other than the name and mailing address, that identifies an individual, including an individual's medical or disability information, but does not include other information related to licensing such as incidents, rules or safety violations, misconduct, training records, commendations, and license status.

The section defines "director," "medical information," "office," "provider of health care" and "unauthorized access."

Defines "personal information," which includes medical information. Defines "medical information" as that which (1) relates to an individual's physical or mental condition, medical history or medical treatment, or (2) is obtained from a medical professional or medical care institution, from the individual, or from the individual's spouse, parent, or legal guardian.

"Personal information" refers to any information maintained by an agency that identifies or describes an individual, including, but not limited to his name, social security number, physical description, home address, home telephone number, education, financial matters, medical history, and employment history. "Record" refers to any file or information about an individual maintained by an agency by reference to an identifying particular such as the individual's name, photograph, finger or voice print, or a number or symbol assigned to the individual.

For purposes of California's adoption of the Interstate Compact on Educational Opportunity for Military Children, "educational records" means: those official records, files, and data directly related to a student and maintained by the school or local education agency, including records encompassing all the material kept in the student's cumulative folder such as general identifying data and health data.
