A health care service plan...contacted by a hospital pursuant to Section 1262.8 (re: transfer of patient for post-stabilization care) shall, within 30 minutes of the call requesting information... discuss the patient's medical record with the treating physician and transmit any appropriate portion of it by fax or email as requested.

Nothing in this section authorizes the further disclosure of confidential medical information ... described in this section by the chief medical examiner-coroner or any of the prehospital emergency medical care personnel except as otherwise authorized by law.
If the victim of abuse consents, a long-term care ombudsman may report an abuse incident to the adult protective services agency or law enforcement agency to get assistance with the investigation. The ombudsman must report health care facility neglect to the bureau and must not identify the victim if the victim refuses consent to being identified in the report. The bureau and reporting agency must maintain the confidentiality of the report.
The medical director of each state hospital for the mentally disordered must make a roster of the gravely disabled persons within the institution. Officer in charge of investigating conservatorships within a county shall be provided with the names and pertinent records of patients to be provided conservatorship investigation.
