"Personal information" refers to any information maintained by an agency that identifies or describes an individual, including, but not limited to his name, social security number, physical description, home address, home telephone number, education, financial matters, medical history, and employment history. "Record" refers to any file or information about an individual maintained by an agency by reference to an identifying particular such as the individual's name, photograph, finger or voice print, or a number or symbol assigned to the individual.
Definition of "confidential public health records": any paper or electronic record maintained by the department or a local health department or agency, or its agent, that includes data or information in a manner that identifies personal information, including, but not limited to, name, social security number, address, employer, or other information that may directly or indirectly lead to the identification of the individual who is the subject of the record.
CalOHii shall assume responsibilities for determining which provisions of state law concerning personal medical information are preempted by HIPAA. State entities impacted by HIPAA shall assist in determining which state laws concerning personal medical information are preempted by HIPAA and conform to all determinations made by the office regarding HIPAA. Any provision of state law pertaining to personal health information that is determined to be preempted by HIPAA shall not be applicable to the extent of that preemption.

Defines "medical information" as any information regarding an individual's medical history, mental or physical condition, or medical treatment or diagnosis by a health care professional. The provision defines "health insurance information" as any unique identifier used by a health insurer to identify the individual, or any information in an individual's application and claims history, including any appeals records.
